Five tables are contained within this mastersheet which record: 1) Record of cases due and dates when final and draft reports are sent out and the date cases are sent to archive. 2) Details of the clinical review, including reviewer, NHS area, date commissioned, date due, date received, dates of escalations, dates of extensions. 3) Data from a pilot process for clinical reviews, including: reviewer, NHS area, commissioned, review level, escalation level, timeliness of review received, was the review on the PPO template, did the review meet needs of investigation, were SMART recommendations made, what questions raised for PPO investigation. 4) Details of discretionary cases not investigated (Case, date the PPO was notified, reason for not investigating) 5) Log of cases where PPO was not informed, Case number, date of death, date discovered.
