This public health intelligence profile describes the patterns in demographics, risk factors and burden of ill health due to long term conditions for people registered with a GP in the South locality. GP localities are clusters of practices working together to share experiences and knowledge, as well as local monitoring of service provision. This profile, and the profile for the North and West localities, have been produced to support this work by identifying needs, patterns, and inequalities at a more local level.
