Each acute hospital or trust should nominate a respiratory clinical lead for discharge care and integrating services, this individual having designated time within their job plan and responsibility for establishing discharge bundles within their organisation, improving the transfer and quality of discharge information to primary care teams, liaising with the existing CCG respiratory programme group or, where such a group is absent, forming one. |
Hospitals should develop an improvement plan, agreed by the MDT and supported formally at Trust board and CCG level, based upon the recommendations within the national and their site-specific report. |
