Sustainability and Transformation Partnerships (February 2017) Names and Codes in EN

A names and codes of sustainability and transformation partnerships (STP) in England as at 14 February 2017. Please note the change in name from plan footprints to partnerships. (14/05/2017).
This includes:

• Name correction for Humber, Coast and Vale, (Sept 2016)
• North Durham CCG move from STP E54000003 to E54000001. Therefore these two STPs have new codes: E54000045 (Durham, Darlington, Teesside, Hambleton, Richmondshire and Whitby) and E54000046 (Northumberland, Tyne and Wear and North Durham). (Dec 2016)
• Correction to the border between the West, North and East Cumbria STP and Lancashire and South Cumbria STP. Therefore these two STPs have new codes: E54000047 (West, North and East Cumbria) and E54000048 (Lancashire and South Cumbria). (Feb 2017)
